Additional Group Information

External / Outside Groups: Upload the Group's Certificate of Insurance and its Expiration Date.

Upload your insurance certificate (PDF or JPG)

Enter expiration date in MM/DD/YYYY format

The Board of Education requires any user to carry liability insurance as to the proposed use in amounts not less thanĀ  $1,000,000 per occurrence/$2,000,000 aggregate combined bodily injury and property damage naming DeKalb School District #428 as an additional insured.
